Reentry & Transition Support

Coming home with a plan — not just a hope.

Overview

Returning to the community after incarceration or institutional care is one of the hardest transitions a person can face. Our Reentry program meets people at that moment with structure, advocacy, and a realistic plan for the first 90 days and beyond.

We coordinate housing referrals, identification and document recovery, compliance with supervision requirements, and the daily structure that turns intention into stability.

  • Create a clear 30/60/90-day reentry plan
  • Stabilize housing, ID, transportation, and basic needs
  • Coordinate with parole, probation, and treatment providers
  • Reduce the risk of reincarceration through accountable structure
Eligibility

Who this is for.

  • Individuals within 12 months pre- or post-release
  • Adults under community supervision (parole, probation, drug court)
  • Referrals from correctional facilities, halfway houses, or attorneys
  • Family members supporting a loved one's return
